In Executive phase, if a new risk has been discovered at first we should submit change request or we can directly add to the risk register?

This risk register, issue log and other transactional non-controlled documents may be added to without a CR.

What do you mean from transactional non-controlled documents?

I mean project documents that are generally NOT under configuration management, NOT baselined. The issue log and risk register I call transactional because we are always adding, updating, deleting from them on a daily basis.

Stakeholder register is another project document that can generally be added to at will by the PM.
