Do you need to submit change requests for both fast-tracking and crashing, or only when those techniques will change the schedule baseline and perhaps other constraints?

You only need to submit change requests when there is a variance between planned vs. actual performance that impact your baselines. In your change request, you can indicate what technique you are using to bring your baselines back.
