Hello, Are there any flow chart to refer about the change management. This will be helpful to under to understand how change log, issue log, request and its approval.

Looking at the PMBOK guide, I am sure you would be able to derive some kind of flow chart.
In real life, companies and PMs may differ on how that process works and flows.
