Yogesh,

First of all, please tell us where you found this question, so that we can give credit to the question creator.

Second, every "good" PMP exam sample question, should have FOUR answers (A, B, C and D). This one only seems to have three, so I would call this a "bad" question just because it doesn't follow the official PMI format.

And third, "Any Stakeholder" can ISSUE a major change request. This is because anyone can have a great idea on how to improve the result of a project, so anyone can ISSUE them. But that doesn't mean that the change request will be implemented. Because before a CR is implemented, someone always has to approved by the appropriate person/group as defined in your change management processes.
