TOPIC: communication management plan update

if any reason led to update communication management plan such as one new stakeholder join , one stakeholder leave the project ,communication requirement of one stakeholder had been changed , so to update communication management plan as it is part of project management plan , change request will be submitted but in such a case is it required to forward it to CCB or project manager can take action and give approval directly for such change requests

Personally I would not require a change request form for the communications plan. I would use document version control to issue a new version with a note saying why it was updated. The kind of change you mention - a new stakeholder - doesn't impact cost/delivery/scope/timelines etc so it wouldn't warrant using the change management process. Your configuration management plan will set out what documents require change control forms and CCB approval.
