If the plan / baseline are not yet approved then you would not need a change request to update that plan. Approved generally means signed off by the relevant stakeholders (usually at least the sponsor). In my experience questions are usually clear about whether the plan or baseline is approved or not.

Change Request

I have a question regarding requirement for Change request in knowledge areas.
Say we are in process of building risk responses, and have updates to one of the plans( Schedule, Cost etc) do we need to raise a change request to update project management plans even though we are in planning stage?
How do we know is they are approved or not ?
