TOPIC: Job Title in PMP application for contractors

Hi,
I am filling PMP application and I want to know what should I fill for the below:

Professional Education -> Course Dates -> Start Date : What date should I put in here? Is it the date I purchased Prepcast? I did purchase it a year back. Will that be an issue?
Job Title : I am a contractor to the client I work. What job title should I put? The company where I work or the title at the client place. Please help....
