First question:
Ad them to the stakeholder register. Complete your analysis and then act accordingly. I would never put a stakeholder on an issue log. Update the Charter if the impact is significant enough: the last time you asked this same question you said they had a high impact.
Second question:
It's a risk: add it to the risk register.

Where did these questions come from so we can give proper attribution?

Could you please let me know am I right in the following cases
First question.
recently in the execution stage, a key stakeholder has been identified, but without conducting a stakeholder analysis, looks she does not make any change in baselines ( scope, time and cost).
What is the next and first action?
1. adding to Identified risk list
2.adding to stakeholder register list
3. Adding to issue log
4. By attaching addendum, add them to the charter
In my opinion, we should consider the following steps:
A. add him in issue log because we aren't sure her effect is positive or negative.
B. after stakeholder analysis add him by attaching an addendum to the charter and stakeholder register.
Second question:
what is the right document to add positive risk at first? :
1. Issue log
2.Risk register
