I always like to quote pg. 2 from the PMBOK, "Because project management needs to be tailored to fit the needs of the project, the standard and the guide are both based on descriptive practices, rather than prescriptive practices. Therefore, the standard identifies the processes that are considered good practices on most projects, most of the time. The standard also identifies the inputs and outputs that are usually associated with those processes. The standard does not require that any particular process or practice be performed."
Essentially, your question presupposes that the PMBOK is a checklist, which it is not. Rather, it is a descriptive in nature and thus allows for tailoring as necessary for your project. So, to answer your question it depends upon the nature of the change. How changes are identified in an IT project are wildly different than how they might occur when building a skyscraper.
