Only the information given below is asked in the PMP exam application. Should I put (PMPREPCAST6) along with the course title and (R.E.P 3023) along with the provider name ?
Also, should I put 60 or 35 hours ? Since the certificate says 60 hours, I put 60 hours.

Please confirm if what I have mentioned below is enough or else please clarify. Thank you.

I believe it is looking different from the PMP application on PMI website since they changed the application in June.
