If risk occurs on one of the project team members, where should we have to document it?

Assuming this risk was previously identified in the planning process:

-You'll have to update the issue log to document what has happened
-Consult with the risk register for risk response implementation, and document the actions you have taken + results
-Log in the lessons learned register.
-Additionally, you may have to submit a change request for any subsequent changes to your resource.
