Can someone please clarify the difference between "project role" and "Job Title" on the PMP application form? In few of my projects I was not the manager but was leading from offshore, can I put "project role" as manager?
Also what is the difference between: Manager, Project manager and Project leader?

And finally what should be the "contact relationship" when someone works with the lead of the project?

Sandeep,

Here is an example to clarify the difference between project role and job title. In my previous position my job title was "Geophysicist". My project role on any particular project varied from being a "project leader" to "project manager". I think in general the difference between project leader and project manager is that the latter typically carries greater overall responsibility. However, in some cases, the project leader is also the project manager so the titles can by synonymous. For example, with my previous employer, we didn't always have formal project managers assigned for specific projects. The person leading the project (and often doing most of the work) was also the "project manager".

"Finally what should be the relationship with the person who one needs to mention as contact point on the application?"

The person you should use as the contact person is the one who can most accurately account for your involvement in the projects you worked on. It could be your supervisor, co-worker or colleague.

Nasara,

In order to be a qualified candidate for the PMP certification you must have experience leading and/or directing all aspects of a project(s). You also must have performed activities across all process groups for at least one of the projects you use for your experience. Please read the PMI handbook carefully as it clearly states the requirements:
