Always list the name of your employer as the "Contact Person" on your application form. This of it this way: If you list 20 customers as your "Contact Persons" and then you get audited, it means that you have to contact 20 customers! But if you list the name of your employer/supervisor, then ONE person can validate the correctness of your application.
And don't forget to show your application to your supervisor BEFORE you submit it, so that they can see it before and can tell you ahead of time if they agree with the numbers you provide or if you need to adjust.
