I'm not sure that it really matters. If I'm talking about a new issue, I would create an issue in the log because it's the first thing on my mind, then I would go back and update the risk register. As long as they both get done around the same time, that would satisfy the process in practical terms. Are you worried about that coming up on the exam as a question? I don't think it would.
A previously identified risk has materialised. So I assume now it becomes an issue. What should the project manager do first?
