The first thing PMI expects you to do when you encounter an issue is to document it - regardless of phase. Issues could be anything from scope disagreements to resourcing issues in Planning. However, as you indicate, this largely depends on the context of the question.

as we know if any issue happen throughout project life cycle the first response from project manager is to record it in issue log to track it and as per PMBOK 6 page 96 which mention that issue log is created for first time as an output of direct and manage project work which is one of processes at executing phase so my question is if any issue identified during planning phase what is the first response from project manager (i.e if the question include planning phase and mention that one issue occurred and question asked about first response from project manager and from answers there is update issue log is this answer is the correct one or not ?)
